PLEASE READ THE BELOW WAIVER PRIOR TO RSVP

In consideration of me, my child, or any person I am registering to participate in the Nancy LaBelle Retirement Party Event, I hereby for myself, my heirs, and personal representatives assume any and all risks that might be associated with the event. I further waive, release, discharge, and covenant not to sue the Down Syndrome Connection of the Bay Area, their programs, officers, employees, sponsors, organizers, volunteers, or other representatives or their successors and assigns, for any and all injuries or damages of any kind whatsoever suffered by myself, my child or any person I am registering for as a result of taking part in the event and any related activities. I also authorize the use by DSCBA and their programs of any photo, film, or videotape taken of me, my child, or any person I am registering for at the event for any purpose. By participating in the event, I voluntarily assume all risk, both known and unknown, related to this event, including any potential exposure to COVID-19. Please note that an inherent risk of exposure to COVID-19 exists in any public place where people are present. COVID-19 is an extremely contagious disease that can lead to severe illness and death. I have read the above notice and warning of health-related exposure, including COVID-19, and agree to the terms described as a condition of participating in this event.
